Hexarelin (examorelin, His-D-2-Me-Trp-Ala-Trp-D-Phe-Lys-NH2) is a synthetic six-amino-acid peptide of the growth hormone-releasing peptide family. Like the other GHRPs it is an agonist of the ghrelin receptor GHS-R1a, but hexarelin is distinctive for a second documented binding partner: the scavenger receptor CD36, expressed on cardiomyocytes, macrophages and endothelium. That dual pharmacology is why hexarelin appears across two separate research literatures: the classic GH secretagogue studies, and a cardiovascular line covering ischemia-reperfusion, heart failure and lung injury models. Why Researchers Study Hexarelin

1. The most potent classical GHRP. In the comparative secretagogue literature hexarelin produces among the strongest GH responses of the hexapeptide family, making it a standard positive control in GHS-R1a assays alongside GHRP-2 and GHRP-6.

2. CD36 pharmacology. Hexarelin is the tool compound of choice for the CD36-PPAR gamma pathway; the Int J Mol Sci review positions it in macrophage cholesterol efflux and metabolic disorder research that no other GHRP reaches.

3. Cardiac injury models. Published rat studies report improved systolic function and reduced inflammatory signaling after ischemia-reperfusion and coronary ligation, with effects blocked by the GHS-R antagonist [D-Lys3]-GHRP-6.

4. Lung injury research. A 2021 mouse study reported improved lung compliance and reduced neutrophil recruitment and collagen deposition in acid-induced lung injury.

Cardiac ischemia-reperfusion, rat (Int Heart J, 2017): hexarelin 100 mcg/kg/day improved systolic function, lowered malondialdehyde and downregulated IL-1 beta signaling; effects were blocked by a GHS-R antagonist and slightly exceeded equimolar ghrelin.
